Cleveland Clinic Develops 10-Year Risk Calculator for Diabetes Complications

Cleveland Clinic has developed a new calculator to predict 10-year risk of developing complications among patients with diabetes and obesity who are deciding whether to receive usual medical care or undergo metabolic surgery.1

“Diabetes can be a devastating disease and combined with obesity, patients are at a high risk for cardiovascular complications. Metabolic surgery is an underutilized treatment for these patients. This calculator can help both physicians and patients quantify the risks and benefits of surgery and make the best decision for treatment,” said Steven Nissen, MD, Chief Academic Officer of the Heart & Vascular Institute at Cleveland Clinic, in a press release.1

Data supporting its effectiveness were collected in 2 phases. In the first phase, Dr Nissen, Ali Aminian, MD, and colleagues compared the effects of metabolic surgery (n = 2287) and usual medical care (n = 11,435) in patients with type 2 diabetes and obesity.2 The findings, which were published in JAMA, indicated that metabolic surgery was significantly associated with a lower risk of incident major adverse cardiovascular events compared with nonsurgical management in this patient population.2

In phase 2, the researchers assessed the same group of patients for potential predictors of various health outcomes.1 They built and integrated evidence-based models into a risk calculator to estimate patients 10-year risk of 26 risk factors, including coronary heart disease, stroke, heart failure, diabetic kidney disease, and mortality, with type 2 diabetes and obesity with and without bariatric surgery.1

The new risk calculator and data on its effectiveness were presented at ObesityWeek 2019, held by the American Society for Metabolic and Bariatric Surgery and Obesity Society in Las Vegas, Nevada. It will be accessible on the Cleveland Clinic Risk Calculator Library and as a smartphone app called “BariatricCalc.”
